John and Betty


John was in the late stages of Parkinson’s disease, receiving hospice services while residing in a skilled nursing facility. His wife, Betty, has diabetes and was living independently in their home. John and Jim, their sons, lived on the east coast and were concerned about their father’s nursing care as well as how well their mother was managing her diabetes, the fact that she was still driving and her increasing forgetfulness.

After completing the assessment process and gaining input from family members and involved medical professionals, we created a plan. We gently added extra services for Betty and John was even able to return home with Betty and with hospice care so he could die peacefully at home.

Throughout the process, we had conversations and reported back to John and Jim who were able to be active participants in decisions and were supported along the way thru this difficult time. They now have peace of mind knowing their father was home in his final days, and their mother has an advocate at AgeWise Advocacy & Consulting.

In addition, we serve as family liaison, a central contact for family members, care providers and other professionals to ensure consistent and effective communication.

Margaret and Stanley


Margaret and Stanley have five children, several of whom are medical professionals. Most family problems are addressed by argument and dissension. As they aged, Margaret began exhibiting symptoms of cognitive decline, and Stanley developed health problems. The children and Stanley found themselves arguing about how to go forward. This stress created so much turmoil for Margaret that she became highly anxious and required psychiatric hospitalization.

The family contacted us, desperate to find some agreement so they could plan for the future and create a nurturing environment for Margaret, to prevent further hospitalizations. We intervened and helped them successfully create a roadmap for decision making.

We will consult with you and your family to help you manage difficult conversations and work with you to develop solutions that are in the best interest of your loved one and your family as a whole.

Mark and Joan


Mark and Joan enjoy living a full and active life. They have lived in the same home for 40 years. Their daughter, Carol, lives on the West coast and has her own business and is supportive, but not easily available. Mark and Joan have a general idea about how they want to live as they age and realize the importance of having a local advocate.

We met with them, collected important information and helped them discuss sensitive topics they have been avoiding which culminated in the creation of a detailed plan for the future. This couple now has a good roadmap and a professional who can advocate on their behalf and be a steadfast resource.
